Payment for Services
Please plan to take care of co-payments, co-insurance and any outstanding balances at the time of service. Co-payments or deductibles must be paid at the time of visit. We accept debit cards, Visa and Mastercard.
If we are not a participating provider with your insurance plan, or your provider does not cover services provided, you will be responsible for any charges incurred. We will provide you a receipt for your insurance company.
Any check returned from the bank will result in an additional $20 charge.
Always notify our office of any change in name, address, phone or insurance information.

Self Pay Policy
If you do not have insurance we require a $132 payment for a first time visit. All appointments thereafter will cost $89 and must be paid at time of service. A 35 percent discount will be applied to all billed charges.
